Fat Grafting: Its Role in Breast Reconstruction

Abstract

Purpose of Review

Fat grafting is a useful adjunct to both implant and autologous reconstruction, in both immediate and delayed settings. More recently it has also been used as a standalone method of breast reconstruction. Herein we discuss the recent literature and experience with autologous fat grafting for breast reconstruction.

Recent Findings

There are multiple methods of fat harvest, and preparation. More recently, new techniques have been developed for larger volume fat grafting, as well as external volume expansion of the breast. In general the procedure is safe with few complications—however, some such as fat necrosis causing a palpable lump and calcifications on imaging can be a source of concern for a patient that has already experienced breast cancer.

Summary

Fat grafting in breast reconstruction, when used appropriately in the properly counselled patient, is a powerful tool. It has an increasing application in breast reconstruction even in the setting of radiation.
