Clinical experience in the treatment of children who swallowed multiple magnetic foreign bodies: A report of five cases

Abstract



Background

Several clinical reports have been published on complications resulting from swallowing multiple magnetic foreign bodies. This study aimed to summarize the clinical experience of managing children who swallowed multiple magnetic foreign bodies.




Methods

We reviewed the clinical records of five children who swallowed multiple magnetic foreign bodies and were admitted to our hospital during June 2012 to June 2014. Details of the patients' presentation, imaging studies, complications and treatment were recorded.




Results

All five children suffered from gastrointestinal perforation and intestinal obstruction. The magnetic foreign bodies caused local bowel wall tissue ischemia necrosis and perforation as well as other complications associated with fistula formation. The magnets were finally removed by laparotomy surgery.




Conclusion

If magnetic foreign bodies cannot be removed by endoscopy, an operation is suggested as soon as possible to avoid serious complications.

Latero-cervical lymph node metastases (N1b) represent an additional risk factor for papillary thyroid cancer outcome

Abstract

Purpose

Papillary thyroid carcinoma (PTC), the most common thyroid cancer histotype, has a good prognosis even when spread to the neck lymph node (LN). The prognostic role of LN metastases' location is still controversial. The aim of the present study was to evaluate the clinical relevance of the number and location of LN metastases at presentation in PTCs.

Methods

This retrospective study included a consecutive series of 1653 PTC patients followed for a mean period of 5.9 years in a referral thyroid cancer clinic. All patients have undergone thyroidectomy with the dissection of at least six LNs. According to the LN status, patients were subdivided into 569 N0 (34.4%), 644 N1a (39.0%, central compartment) and 440 N1b (26.6%, latero-cervical compartment).

Results

Age at diagnosis was significantly lower in N1b (39.8, IQR 30.7–51.6) and N1a (40.1, IQR 31.3–50.1) than in N0 (44.7, IQR 36.6–55.0 yrs). The male gender was more prevalent in N1b than in N1a and N0 (F/M = 1.9/1, 4.0/1 and 5.5/1, respectively). Persistent/recurrent disease at last control was significantly more frequent in N1b (29.8%) than in N1a (14.3%), and in N1a than in N0 (4.2%) (p < 0.01 for all). Also, distant metastases were significantly (p < 0.001) more frequent in N1b (14.1%) than in N1a (4.3%) and N0 (1.6%). LN metastases' number (>5) was a significant risk factor for persistent/recurrent disease only for N1a patients.

Conclusions

These data indicate that persistent/recurrent disease and distant metastases are significantly more frequent in patients with latero-cervical LN (N1b) metastases and that the LN location should be used for a better postsurgical risk stratification.

A randomised trial of a barrier lipid replacement strategy for the prevention of atopic dermatitis and allergic sensitisation: The PEBBLES Pilot Study

Abstract

It is hypothesised that the impaired skin barrier in atopic dermatitis (AD) allows the immune system to be exposed to environmental allergens, resulting in sensitisation and allergic disease[1]. Two small trials recently found that routine use of emollients reduced the incidence of AD during the active treatment period by approximately half [2, 3]. It remains unknown if prophylactic use of emollients can prevent the development of AD beyond the treatment period (as opposed to simply delay its onset) or if this reduction in AD leads to a reduced risk of allergic sensitisation.

Cancer Stem Cells in Hepatocellular Carcinoma

Abstract

Background

Hepatocellular carcinoma is one of the most common cancers and the second leading cause of cancer-related deaths worldwide. Only a small proportion of patients benefit from curative treatment and the prognosis is very poor for the majority of cases due to late presentation, resistance to chemotherapy and high recurrence rate. In recent years, progress in stem cell biology allowed us to explain that hierarchically organized cancer stem cells (CSCs) drive histological and functional heterogeneity of hematological malignancies and solid tumors.

Methods and Results

Also referred to as tumor-initiating cells, CSCs have been isolated from both hepatocellular carcinoma (HCC) cell lines and primary tumors by using hepatic progenitor markers. Although there is still no consensus on cancer stem cell phenotype in HCC, single or combined use of CSC markers defines a minor population of tumor cells with the capacity of self-renewing and the ability to recapitulate the original tumor heterogeneity.

Conclusions

This review focuses on the biological features of CSCs and their potential as diagnostic/prognostic tools and therapeutic targets in HCC.

A polyamide thin-film composite membrane modified by Michael addition grafting of hyperbranched poly(amine ester)

Abstract

In this work polyamide thin-film composite membrane (TFC) surface modified via Michael addition grafting of a hydrophilic hyperbranched poly(amine ester). For this purpose, amine-rich polyamide layer formed by interfacial polymerization on a polyethersulfone support, and then acrylated hyperbranched poly(amine ester) (AC-HBPAE) was used as grafting moiety. The membrane surface was characterized by attenuated total reflectance Fourier transform infrared spectroscopy (ATR-FTIR), atomic force microscopy (AFM) and water contact angle techniques. Field emission scanning electron microscopy (FE-SEM) was used to evaluate surface and cross-section morphology of samples. Filtration performances and bio-fouling resistance were also studied using a nanofiltration cell. Surface chemical composition and contact angle indicated the successful grafting of acrylated poly(amine ester) to the membrane surface. The results also indicated there is a solid relationship between acrylation percentage of hyperbranched polymer and membrane properties such as fouling resistance. A uniform and hydrophilic surface observed for TFC membrane modified with 5% acrylated hyperbranched poly(amine ester).
