The association between drugs and bullous pemphigoid

Abstract

Drug-induced bullous pemphigoid (DIBP) is often poorly defined in the existing literature from the classic, spontaneous form of bullous pemphigoid (BP). The pathogenesis is unclear and the association between bullous pemphigoid and various medications is tenuous1-2.Commonly reported associations include neuroleptics3-4and diuretics3, 5 with more recent literature drawing attention to dipeptidyl peptidase-4 inhibitors6.Strength of drug causality associations is generally determined by factors such as latency between drug initiation and onset of disease, epidemiological risk, effects of withdrawal/re-challenge. Prior drug association studies were based on broader inclusion criteria and may overestimate the true association3-6.

Hydroxychloroquine (HCQ) is an effective drug that is widely used to treat connective tissue and autoimmune diseases. Overall, it has an excellent safety profile; however, its most potentially serious and irreversible side-effect is retinal toxicity. The Royal College of Ophthalmologists (RCO) published their recommendation on HCQ and ocular toxicity screening in 2009.

An experimental study of adolescent sleep restriction during a simulated school week: changes in phase, sleep staging, performance and sleepiness

Summary

This laboratory study investigated the impact of restricted sleep during a simulated school week on circadian phase, sleep stages and daytime functioning. Changes were examined across and within days and during a simulated weekend recovery. Participants were 12 healthy secondary school students (six male) aged 15–17 years [mean = 16.1 years, standard deviation (SD) = 0.9]. After 2 nights with 10 h (21:30–07:30 hours), time in bed was restricted to 5 h for 5 nights (02:30–07:30 hours), then returned to 10 h time in bed for 2 nights (21:30–07:30 hours). Saliva was collected in dim light on the first and last sleep restriction nights to measure melatonin onset phase. Sleep was recorded polysomnographically, and the Psychomotor Vigilance Task (PVT) and Karolinska Sleepiness Scale were undertaken 3-hourly while awake. Average phase delay measured by melatonin was 3 h (SD = 50 min). Compared to baseline, sleep during the restriction period contained a smaller percentage of Stages 1 and 2 and rapid eye movement (REM) and a greater percentage of Stage 4. PVT lapses increased significantly during sleep restriction and did not return to baseline levels during recovery. Subjective sleepiness showed a similar pattern during restriction, but returned to baseline levels during recovery. Results suggest that sustained attention in adolescents is affected negatively by sleep restriction, particularly in the early morning, and that a weekend of recovery sleep is insufficient to restore performance. The discrepancy between sleepiness ratings and performance may indicate a lack of perception of this residual impairment.

Meat tenderness appears as top rated issue to be solved concerning meat sensorial quality, which requires enhanced knowledge and further work to understand the processes involved. Meat tenderness plays an important role, where entire pieces of meat are cooked, fried or barbecued. In these cases some types of meat, in particular beef, have to undergo a certain ripening or ageing period before cooking and consumption in order to achieve the necessary tenderness. In the fabrication of many processed meat products the toughness or tenderness of the meat used is of minor importance. Many meat products are composed of comminuted meat, a process where even previously tough meat is made palatable. Further processing of larger pieces of meat (e.g. raw or cooked hams) also results in good chewing quality as these products are cured and fermented or cured and cooked, which makes them tender. Tenderness of meat depends on various pre-and post slaughter factors. In the recent past, much of the advancements were occurred in the methods employed for tenderization of meat. These newer methods of meat tenderization resulted in better efficiency and high quality meat products. In this review, tenderness of meat and their mechanism, methods of tenderization and their evaluation techniques have been discussed briefly

Studies were conducted in the Ngaoundere Municipal abattoir from May 2012 to March 2013 to have an insight on diseases of cattle slaughtered. Ante mortem and post mortem examinations were done on a total of 4072 cattle during eighty two visits at a rate of twice per week on randomly selected days. The results showed that 93.2 % of slaughtered cattle were cows. Tick infestation (33.3 %), dermatophilosis (13.7 %) and fatigue (3 %) were the major health conditions observed following ante mortem examination.
