ABSTRACT

Vertigo and balance disorders in Meniere's disease (MD) may have various etiology. The aim of the review is discussing pathogenetic mechanisms of the typical vertiginous paroxysms in MD, resulting from endolymphatic hydrops as well as analysis of etiology, pathogenesis, clinical course and basic treatment of paroxysmal and permanent forms of vertigo and balance disorders, caused by other conditions, associated with MD. We discussed the course of MD complicated by vestibular migraine, benign positional paroxysmal vertigo, functional dizziness, bilateral vestibulopathy and vestibular drop-attacks.

Small salivary glands in the paratonsillar space in children

ABSTRACT

Was to study the changes in the small salivary glands in chronic tonsillitis and paratonsillar abscess in children. A histological examination of the palatine tonsils of 22 children aged 5 to 14 years old who had a paratonsillar abscess and 18 children suffering from chronic tonsillitis was carried out. Fragments of small salivary glands, located both superficially, in the mucous membrane between the stratified squamous non-keratinizing epithelium and the lymphoid tissue of the palatine tonsils, and between the muscles of the amygdala were found in 42 of the 80 (52.5%) tonsils studied. Along with the unchanged glandular structure, 26 (61.9%) samples revealed areas of destruction foci in small salivary glands with signs of inflammatory and compensatory regenerative activity. The presence of separate interlobular ducts of the salivary glands with pronounc ed periductal lymph - macrophage infiltration was noted. The subepithelial lymph-macrophage infiltrates found in the lamina propria are associated with the exit of the excretory ducts of the salivary glands to the surface of the mucous membrane. Many thin-walled veins are surrounded by perivascular lymphocellular infiltrates. The inflammatory and post-inflammatory changes in the small salivary glands revealed by us in children with chronic tonsillitis do not exclude their participation in the pathogenesis of paratonsillitis. At the same time, inflammation in the salivary gland can be both primary, as a result of its infection with live microorganisms from the tissues of the tonsil, and secondary, due to the compression of the excretory ducts by the edematous tissue of the tonsil in acute tonsillitis.

ABSTRACT

Neoplasms of the upper respiratory tract and ear are tumors of visual localization; however, patients often go to specialized medical institutions in the presence of a widespread tumor process. This paper presents a brief overview of the most common benign and malignant tumors with localization in the oropharynx, nasopharynx, larynx and ear. The clinical picture, diagnosis and various treatment options are presented. Attention is paid to early diagnosis and the need for timely consultation with an oncologist and doctors of related specialties.

Clinical observation of a patient with bilateral tuberculous otitis media developing against the background of HIV infection

ABSTRACT

The lesion of middle ear as a variant of clinical manifestation of secondary tuberculosis, according to international scientific literature, is a rare condition. However, in terms of real clinical practice, the proportion of this pathology should not be underestimated due to the wide spread of tuberculosis infection among the population of developing countries, including the Russian Federation. One of the risk factors for the systemic spread of mycobacterial flora is a concomitant HIV infection, which often acts as an opportunistic pathological agent for tuberculosis. The treatment of such a state is always challenging because of the high aggressiveness of bacteria and the low immunological resistance of the patient. This article presents a clinical case of bilateral tuberculosis otitis media, which was a manifestation of tuberculosis infection that dev eloped on the background of HIV infection.

ABSTRACT

Review of articles published in peer-reviewed international journals devoted to the issues of rehabilitation of patients with peripheral vestibular dysfunction using virtual reality conditions. IN the review provides a neurophysiological rationale for the use of virtual reality techniques for rehabilitation, the technical methods used to recreate the virtual environment, as well as the published results of their clinical application. It is concluded that the application of virtual reality methods in clinical vestibulology is promising. The search was carried out using the search engines Google Scholar, PubMed, PMC, CrossRef, Science Direct.

The association between aeroallergenic structures and allergic rhinitis: a study on northern Vietnam

ABSTRACT

Recently, there has been an increase in the number of patients with allergic rhinitis (AR) and the number of publications devoted to this problem is increasing. The main etiological factors of AR are pollen of trees, meadow and weeds, as well as mold spores, household allergens and epidermis of animals. Epidemiological studies have found that the prevalence and structure of AR are influenced by regional characteristics, such as the climatic and geographical and social characteristics of the region, and successively therapeutic and preventive algorithms in AR are also different.

AIM: To examine the phenotype of the incidence of AR in connection with the characteristics of aeroallergens under the influence of climatic and geographical conditions in northern Vietnam, to make a new contribution to knowledge about AR in Asia and to increase the effect of treatment and prevention in this territory.

MATERIALS AND RESEARCH METHODS: The study was conducted in the period from 06.2018 to 09.2018 on the basis of the ENT department of Thainguyen Central Hospital, Vietnam. A total of 556 patients with pathology of ENT organs aged 18 to 70 years were examined, 158 cases of chronic rhinitis were revealed. Among data from 158 patients, 64 patients were diagnosed with AR. We used otorhinolaryngological examination, a standard specific allergological examination and carried out aeropolyneological research in the city of the northern Vietnam, from 06.2018 to 06.2019.

RESULT: The aerobiological spectrum is dominated by pollen from the families Moraceae, Urticaceae, Poaceae, Acacia, Artemisia, fern spores and fungal spores from the genus Alternaria. Among patients with chronic rhinitis, 40% were diagnosed with AR, 98.44% of them year-round or perennial AR, with predominant sensitization to house dust mites and molds, much more often t o plant pollen. Among 9 (14.06%) patients diagnosed with a polyp of the nasal cavity, 6 (9.37%) patients had increased levels of specific IgE in the blood to a mixture of molds. Sensitization in patients with AR with hypertrophy of the mucous membrane of the nasal cavity is predominant on house dust.

CONCLUSION: Predominantly, AR in northern Vietnam was year-round. Especially the connection between the formation of a polyp of the nasal cavity and hypersensitivity to fungal spores has been indicated, which may also indicate the role of social factors in further recommendations for the diagnosis, treatment and prevention of AR in patients living in North Vietnam.

Herbal medicine for acute sinusitis - current trends

ABSTRACT

THE AIM OF THE STUDY: Was to summarize data on the role and place of herbal remedies, in particular, the extract from the roots of Pelargonium sidoides (EPs 7630) in the treatment of acute sinusitis.

MATERIAL AND METHODS: Materials of scientific publications included in the Cochrane Library, information bases of the RSCI, MEDLINE, PubMed were used as a data source. The choice of material was carried out according to the keywords: epidemiology of acute sinusitis, modern treatment of acute sinusitis, colds, herbal remedies, Pelargonium sidoides.

RESULTS: Analysis of published clinical trial data demonstrates evidence that EPs 7630 is significantly superior to placebo in reducing the duration and severity of symptoms of acute respiratory infection and indicates the advisability of including an extract from Pelargonium sidoid es (EPs 7630) roots in OS treatment protocols. The high safety profile and clinically significant efficacy of this drug make EPs 7630 an important component of drug therapy for acute sinusitis.

Psychoacoustic tests for perceptual assessment of processor fitting in patients with cochlear implants

ABSTRACT

THE AIM OF THE STUDY: Was to study the possibility of using a battery of psychoacoustic tests to assess the tuning of the cochlear implant processor (CI) in deaf patients.

METHODOLOGY: The study involved 60 prellingually deaf patients aged 10 to 23 years with oral speech skills. To assess the quality of the CI processor tuning, in addition to traditional methods, a special battery of psychoacoustic tests was used. The first block of tests assessed the perception of the basic characteristics of sound signals (duration, temporal structure, spectrum, timbre) and was used to assess the initial setting. The second block of tests, intended for patients with experience using CI, included tasks to distinguish acoustically similar and dynamically changing signals, etc.

RESULTS: At the end of the initial CI setup session, patients with short signal pe rception problems were identified. Adjusting the frequency of electrical stimulation in patients has increased their ability to distinguish between sounds. During the second tuning session of the CI processor, 6 months later, a group of patients with difficulties in perceiving acoustic information in the low-frequency range was identified - distinguishing melodic intervals, changing the pitch of sounds, highlighting the voice of the target speaker. The «problem» patients underwent additional correction of the CI processor setting and the corresponding auditory training, which improved the test performance and subjective perception of sounds.

CONCLUSION: The use of psychoacoustic tests expands the possibilities of fine tuning the CI processor, taking into account the individual characteristics of the patient's auditory perception at different stages of CI use, especially in «problem» patients.

The history of the antibody as a tool

ABSTRACT

Antibodies are essential tools in modern science and medicine, however the history leading to the use of antibodies as tools has not been well-described. The objective of this paper is to analyze the history of immunology from smallpox inoculation to the production of monoclonal antibodies, and to identify turning points in immunological theory leading to the emergenc e of antibody-tools. In the early 1700's, Western medicine adopted smallpox inoculation from Turkey, along with the idea of acquired immunity. The Germ Theory of disease had to replace spontaneous generation and miasma theory in the 1880's, however, before inoculation could successfully be applied to other diseases. Inquiry into acquired immunity led to the idea of the "antibody" in the 1890's, and the use of antiserum to identify bacteria. Immunostaining was invented in 1942 by repurposing antibody-dye conjugates originally intended as antibiotics. Monoclonal antibody-producing hybridomas were similarly invented in 1975 by repurposing techniques from virology and genetics.

A Rare Case of Laryngeal Hamartoma

Abstract

Hamartoma presenting in the larynx is a rare entity. The common presentation of such a lesion may include stridor, hoarseness of voice, choking sensation, dyspnea and in children, failure to thrive. It is a benign lesion and can be confused with other benign lesions like lipoma, fibroma, chondromas etc. Pathologically it consists of a mesodermal compartment and a granular compartment. Hamartomas mainly occur  in pediatric age group. When it occurs  in adults, males are more commonly affected than females. With  very few cases reported so far, we present a case of laryngeal hamartoma presenting in a lady with progressive hoarseness of voice as a presenting feature.
